Enhancement Coincides with Alexandria, Va., SNF Earning CMS Five-Star Quality Measures Rating

ALEXANDRIA, Va. (Sept. 7, 2023) Alexandria Rehabilitation & Healthcare Center has launched a new, comprehensive Chronic Illness Support model, partnering with Capital Caring Health Hospice to integrate palliative care, pastoral support and other services with its specialized clinical programming. The programming enhancement coincides with the Alexandria skilled nursing facility earning a five-star rating for quality measures from the Centers for Medicare & Medicaid Services (CMS). 

Located at 900 Virginia Avenue, Alexandria provides post-hospital care, short-term rehab and long-term residential care. “Alexandria’s new program for chronically ill patients is truly a next-level model of care, and it is being supported by an excellent partner in Capital Caring Health,” said Marquis’ Megan Shilling, regional director of market development for Marquis Health Consulting Services, which supports the facility. “There is great value in providing a holistic focus on both physical and spiritual wellness for patients.”      

Alexandria developed its patient-centric Chronic Illness Support initiative based on a platform introduced by Marquis. The palliative care component of the program is administered by a trained team of doctors, nurses and palliative specialists, and focuses on providing expert medical care that includes emotional and spiritual support to manage chronic conditions and to help relieve symptoms and stress. The goal is to enhance quality of life for patients and their loved ones. 

Alexandria’s Chronic Illness Support initiative supports its range of specialty programming, including Cardiac Care and Pulmonary Care, which features full time respiratory therapy. It also weaves in disease-specific education and health coaching, multi-specialist led and coordinated care, social services support and connections to community resources and life enrichment activities.    

Top Rating Reflects Change and Progress

The Chronic Illness Support program establishment and the CMS five-star rating for quality measures come at a time of notable change and progress for Alexandria Rehabilitation & Healthcare Center (formerly Envoy of Alexandria). The center is also undergoing a multi-million-dollar renovation and programming enhancement following an ownership change.

As part of the U.S. Department of Health & Human Services, CMS is responsible for quality standards in long-term care facilities through its survey and certification process: it gives each nursing home a rating of between one and five stars. Facilities with five stars are considered to have above average quality.
