Harrisburg, Pa., SNF Partners with Leading Area Providers for New Specialty Initiatives

HARRISBURG, Pa. (May 21, 2024) Capitol Rehabilitation & Healthcare Center in Harrisburg has introduced two new, in-facility specialty healthcare offerings, partnering with top area providers. The 138-bed skilled nursing facility now offers Cardiac Care under the direction of board-certified cardiologist Rajesh Davé, MD, and Chronic Illness Support services featuring palliative care through Hospice of Central PA. 

Located at 4000 Linglestown Road, Capitol provides post-hospital care, short-term rehab and long-term residential care. “The introduction of Cardiac Care and Chronic Illness Support services at Capitol further builds on the center’s evolution as a leading regional specialized services provider,” said Nikki Gachot, regional director of market development for Marquis Health Consulting Services, which supports the center.  

Cardiac Care Program 

Capitol’s Cardiac Care program focuses on promoting independence and quality of life through reducing symptoms and decreasing disability in heart patients. It is designed to assist individuals suffering from challenging or life-threatening heart conditions, including those recovering from a cardiac event or surgical procedure, and those at risk for cardiovascular disease.   

Dr. Davé, who is triple board certified in cardiovascular disease, interventional cardiology and endovascular medicine, was recently named the 2024 Physician of the Year by Central Pennsylvania Business Journal. He serves as president of the Surgical Center of Cardiovascular Experts of Pennsylvania. “Dr. Davé is a recognized leader and innovator in cardiology; Capitol is fortunate to have him in this clinical leadership role,” noted Gachot. 

Chronic Illness Support Model 

Capitol’s patient-centric Chronic Illness Support program integrates palliative care, pastoral support and other services with the center’s clinical specialized programming. Based on a platform developed by Marquis, the initiative focuses on providing emotional and spiritual support to help relieve symptoms and stress for patients and loved ones to enhance quality of life. Christina Ring, CRNP, a board-certified practitioner with Hospice of Central PA’s Levos Palliative Care team, is leading the Capitol program for her organization. 

Capitol’s Chronic Illness Support model also weaves in disease-specific education and health coaching, multi-specialist led and coordinated care, social services support and connections to community resources. “Capitol’s program for chronically ill patients is truly a next-level model of care, and it is being supported by an excellent partner in Hospice of Central PA,” added Gachot.
