Chronic Illness Support Model at Philadelphia Skilled Nursing Facility Supports Specialty Programming 

PHILADELPHIA (Oct. 25, 2023) Logan Square Rehabilitation & Healthcare Center in Philadelphia has introduced a new, comprehensive Chronic Illness Support care model, partnering with Jefferson Health to integrate palliative care, pastoral support and other services with its specialized clinical programming. Located at 2 Franklin Town Blvd., the skilled nursing facility provides post-hospital care, short-term rehab and long-term residential care. 

The palliative care component of Logan Square’s Chronic Illness Support program is administered by a trained team of doctors, nurses and palliative specialists and focuses on providing expert medical care that includes emotional and spiritual support to manage chronic conditions and to help relieve symptoms and stress. The goal is to enhance quality of life for patients and their loved ones.    

The overall initiative is specifically supporting Logan Square’s range of specialty programming, including Cardiac Care and Pulmonary Care, which features full-time respiratory therapy. It also weaves in disease-specific education and health coaching, multi-specialist led and coordinated care, social services support and connections to community resources and life enrichment activities.  

“Logan Square and Jefferson are at the forefront of bringing palliative care to the post-acute skilled nursing setting – it’s very exciting,” said John Liantonio MD, MBA, HMDC, FAAHPM, who serves as the director of palliative care at Jefferson Health University Hospital and is overseeing the initiative at Logan Square. “With this partnership, palliative care specialists are meeting patients where they are in their care journey and providing appropriate services, often when they need it most.”

Of particular note, Dr. Liantonio noted that patients at Jefferson Health can now receive the continuum of palliative care services as they transition to the skilled nursing setting. Jefferson Health’s Hospice and Palliative Medicine Fellowship Program, which Dr. Liantonio leads, further reflects its commitment to palliative care. Fellowship participants will have the opportunity to be part of the initiative at Logan Square. 

Logan Square developed its patient-centric Chronic Illness Support program based on a platform introduced by Marquis Health Consulting Services, which supports the facility. “Logan Square’s new program for chronically ill patients is truly a next-level model of care, and it is being supported by an excellent partner in Jefferson,” said Marquis’ Danielle Koons, senior regional director of market development. “There is great value in providing a holistic focus on both physical and spiritual wellness for patients.”    

Logan Square Rehabilitation & Healthcare Center is part of the only Continuing Care Retirement Community (CCRC) in Philadelphia’s Center City district. The campus – which is in the midst of an expansive renovation also features Fountain View at Logan Square, a luxury independent senior living high rise, as well as assisted living apartments.
