Grant Program Explores Palliative Care Interventions for Dementia Patients

CATONSVILLE, Md. (Nov. 2, 2022) Meadow Park Rehabilitation & Healthcare Center is partnering with the University of Maryland’s School of Social Work and Gilchrist in a National Institutes of Health and Aging (NIH/NIA) funded program studying palliative care interventions for long-term care residents with dementia. The 120-bed skilled nursing facility in Catonsville recently launched its participation in the “Utilizing Palliative Leaders in Facilities to Transform Care for Alzheimer’s Disease” (UPLIFT) Program. 

Palliative care is a specialized medical model for people living with a serious, chronic or life-limiting illness. With a holistic approach, it focuses on providing the necessary medical, emotional and spiritual support to help relieve symptoms and stress.  

The UPLIFT Program will evaluate outcomes of palliative care in enhancing quality of life and extending life expectancy of long-term care residents with a diagnosis of mild to severe dementia. At Meadow Park, palliative care professionals from Gilchrist and the University of Maryland’s School of Social Work are working with the skilled nursing facility’s clinical team in implementing the program. 

“Skilled nursing facilities are a critical site of care for those with Alzheimer’s and related dementias,” said Nikki Gachot, regional director of market development for Marquis Health Consulting Services, which supports the facility. “Meadow Park is honored to partner with Gilchrist and UMD – leaders in their respective fields – in further enhancing support for those dealing with a dementia diagnosis.” 

Meadow Park plans to have approximately 40 long-term care residents participate in the UPLIFT initiative over the course of five years. Seven other skilled nursing facilities are involved in the program; participating facilities receive a grant total of $15,000.  

Established in 1961, the University of Maryland’s School of Social Work has grown to become one of the largest and most respected schools of social work in the nation. Gilchrist, Maryland’s leading nonprofit provider of serious illness and end-of-life care, provides a range of services, including elder medical care, counseling and hospice. 

Located at 1525 N Rolling Road, Meadow Park provides post-hospital care, short-term rehab and long-term residential care. The facility recently underwent a $5 million renovation and modernization, featuring top-to-bottom upgrades, including the expansion of its state-of-the-art rehab therapy gym.
