Catonsville, Md., Skilled Nursing Facility Partnering with Palliative Care Specialist Gilchrist in New Initiative

CATONSVILLE, Md. (May 18, 2023) – Meadow Park Rehabilitation & Healthcare Center in Catonsville has introduced a new, comprehensive Chronic Illness Support model, partnering with Gilchrist to integrate palliative care, pastoral support and other services with its specialized clinical programming. Located at 1525 N Rolling Road, Meadow Park provides post-hospital care, short-term rehab and long-term residential care.
Meadow Park developed its patient-centric Chronic Illness Support program based on a platform introduced by Marquis Health Consulting Services, which supports the facility. “Meadow Park’s new program for chronically ill patients is truly a next-level model of care, and it is being supported by an excellent partner in Gilchrist,” said Marquis’ Nikki Gachot, regional director of market development. “There is great value in providing a holistic focus on both physical and spiritual wellness for patients.”
The palliative care component of Meadow Park’s Chronic Illness Support program is administered by a trained team of doctors, nurses and specialists and focuses on providing emotional and spiritual support to help relieve symptoms and stress. The goal is to enhance quality of life for patients and their loved ones. Gilchrist is Maryland’s leading nonprofit provider of serious illness and end-of-life care.
Meadow Park’s overall initiative is specifically supporting its Chronic Kidney Disease (CKD) management and Cardio Pulmonary Care programs. It also weaves in disease-specific education and health coaching, multi-specialist led and coordinated care, full time respiratory therapy, social services support and connections to community resources and life enrichment activities.
Committed to introducing a range of progressive skilled nursing care services to the region, Meadow Park leverages established partnerships with leading healthcare providers and health systems. To that end, the facility also partnered with Gilchrist and the University of Maryland’s School of Social Work in a National Institutes of Health and Aging (NIH/NIA) funded program studying palliative care interventions for long-term care residents with dementia.
