Specialty Initiative at Catonsville, Md., Skilled Nursing Facility Enhances Rehab Services

CATONSVILLE, Md. (Oct. 5, 2023) – Meadow Park Rehabilitation & Healthcare Center in Catonsville welcomes Mark Tolentino, MSN, FNP-C, of Comprehensive Rehab Consultants (CRC) as its new physiatry clinician. Located at 1525 N Rolling Road, Meadow Park provides post-hospital care, short-term rehab and long-term residential care.
Physiatry services – also known as physical medicine and rehabilitation (PM&R) services – blend pain treatments and physical therapy with the goal to help patients avoid surgery. This “whole body” approach at Meadow Park focuses on designing, coordinating and overseeing customized rehabilitation therapies for patients while managing their medical conditions, including co-morbidities.
Tolentino brings a strong background in physiatry and wound care in skilled nursing settings to his new role at Meadow Park. He is a licensed Advanced Practice Registered Nurse and a certified Family Nurse Practitioner.
Tolentino’s appointment comes on the heels of Meadow Park’s completion of a $5 million renovation and modernization, featuring top-to-bottom updates. The upgrade featured the expansion of its state-of-the-art rehab therapy gym.

Physiatry services are known to significantly decrease both the incidence of major falls with injury and of rehospitalizations for patients, as well as reduce their pain levels, noted Tolentino. “It’s transformative skilled nursing care,” he added. “I’m excited to join the talented team here at Meadow Park as we focus on providing for the best possible outcomes for our patients.”
In addition to in-facility physiatry services, Meadow Park rehab patients benefit from the support of Comprehensive Rehab Consultants after discharge. Care coordination nurses help in ordering equipment, understanding medication and coordinating follow-up appointments. CRC is a physician-owned medical group, operating in more than 25 states.
