Langhorne, Pa., Skilled Nursing Facility Care Model Emphasizes Physical and Spiritual Wellness 

LANGHORNE, Pa. (July 18, 2023) — Enhancing its commitment to progressive skilled nursing care, Oxford Rehabilitation & Healthcare Center in Langhorne has introduced a new, comprehensive Chronic Illness Support model, partnering with Bridges Palliative Care to integrate palliative care and pastoral support with its specialized clinical programming.  

Oxford developed its patient-centric Chronic Illness Support care model based on a platform introduced by Marquis Health Consulting Services, its administrative and consulting services provider. “Oxford’s specialized care initiative emphasizes a holistic focus on both physical and spiritual wellness for chronically ill patients – it is truly a next-level model of care supported by an excellent partnership with Bridges,” said Marquis’ Erin Popelak, regional director of market development.   

The Oxford Program – which specifically supports the facility’s physician-led Pulmonary Care Program – incorporates palliative care, which is administered by a trained team of doctors, nurses and specialists. It focuses on providing emotional and spiritual support to help relieve symptoms and stress, and also weaves in disease-specific education and health coaching, social services support, life enrichment activities and connections to community resources.  

“Oxford for years has provided exceptional care for those with chronic disease through its physician-lead specialty offerings,” noted Popelak. “The facility’s new Chronic Illness Support model introduces a palliative care-focused overlay to further improve mental health and well-being for these patients and their loved ones. Ultimately, the program will support improved health outcomes and reduced hospitalizations within this patient population.”    

Located at 300 E. Winchester Ave., the skilled nursing facility provides post-hospital, short-term rehab and long-term residential care. Oxford’s introduction of a new Chronic Illness Support model coincides with a significant upgrade underway at the facility, which maintains a five-star rating from the Centers for Medicare & Medicaid Services (CMS). Oxford Rehabilitation & Healthcare Center is part of the Oxford Enhanced Senior Living campus, which also includes Oxford Crossings, a personal care component featuring a memory care unit and independent living apartments. In total, Oxford Enhanced Senior Living offers 415 skilled nursing and personal care units.
