Programming Enhancement Reflects Positive Momentum at Richmond SNF
RICHMOND, Va. (June 29, 2023) — Southampton Rehabilitation & Healthcare Center in Richmond has introduced a comprehensive Chronic Illness Support care model, partnering with Affinity Care of Virginia Hospice and Palliative Care to integrate palliative care, pastoral support and other services with its specialized clinical programming. The partnership launch comes at a time of positive momentum for the 196-bed skilled nursing facility, where a multi-million-dollar renovation and modernization is planned following a recent rebranding.
Located at 7246 Forest Hills Avenue, Southampton provides post-hospital care, short-term rehab and long-term residential care. The facility developed its patient-centric Chronic Illness Support model based on a platform introduced by Marquis Health Consulting Services, its administrative and consulting services provider.
“Southampton’s new program for chronically ill patients is truly a next-level model of care, and it is being supported by an excellent partner in Affinity,” said Marquis’ Megan Shilling, regional director of market development. “There is great value in providing a holistic focus on both physical and spiritual wellness for patients.”
The palliative care component of Southampton’s Chronic Illness Support program is administered by a trained team of doctors, nurses and palliative specialists. It focuses on providing expert medical care that includes emotional and spiritual support to manage chronic conditions and help relieve symptoms and stress. The goal is to enhance quality of life for patients and their loved ones.
The overall initiative is specifically supporting Southampton’s Physiatry services offering, and will integrate with additional specialty care programs currently in development. The Chronic Illness Support model also weaves in disease-specific education and health coaching, multi-specialist led and coordinated care, social services support and connections to community resources, and life enrichment activities.
According to Shilling, the upcoming renovation will touch on every part of Southampton, including the expansion and modernization of the facility’s therapy gym. Marquis will spearhead the head-to-toe upgrade. “Southampton is focused on becoming a destination of choice for progressive skilled nursing,” said Shilling. “The addition of the Chronic Illness Support model is a clear reflection of that.”
Brick, N.J-based Marquis Health Consulting Services provides administrative and consulting services to skilled nursing facilities and senior housing communities along the Eastern Seaboard.
