Skilled Nursing Facility’s Patient-Centric Approach Emphasizes Physical and Spiritual Wellness

SPRINGFIELD, Pa. (Oct. 26, 2022) — Enhancing its commitment to progressive skilled nursing care,Springfield Rehabilitation & Healthcare Center in Springfield has introduced a new, comprehensive Chronic Illness Support model, partnering with Bridges Palliative Care to integrate palliative care, pastoral support and other services with its specialized clinical programming. The facility provides post-hospital care, short-term rehab and long-term residential care. 

Springfield developed its patient-centric Chronic Illness Support program based on a new platform recently introduced by Marquis Health Consulting Services, which supports the facility. “Springfield’s specialized care initiative emphasizes a holistic focus on both physical and spiritual wellness for chronically ill patients – it is truly a next-level model of care supported by an excellent partnership with Bridges,” said Marquis’ Danielle Koons, senior regional director of market development.  

The Chronic Illness Support program incorporates palliative care, which is administered by a trained team of doctors, nurses and specialists, and focuses on providing emotional and spiritual support to help relieve symptoms and stress. The goal is to enhance quality of life for patients and their loved ones. The overall initiative – which is specifically supporting Springfield’s Pulmonary Care program – also weaves in disease-specific education and health coaching, multi-specialist led and coordinated care, full time respiratory therapy, social services support, and connections to community resources and life enrichment activities.  

“Springfield for years has provided exceptional care for those with chronic disease through its physician-lead specialty offerings,” noted Koons. “The facility’s new Chronic Illness Support model introduces an added layer to further improve mental health and wellbeing for these patients and their loved ones. The program will support improved health outcomes and reduced hospitalizations within this patient population.”  

Located at 463 West Sproul Road, Springfield Rehabilitation & Healthcare Center is in the midst of a multi-million-dollar upgrade. The facility was recognized as a 2021 recipient of the Bronze – Commitment to Quality Award by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL), the nation’s largest association of long term and post-acute care providers. In addition, Springfield was recognized as a U.S. News & World Report “Best Nursing Home 2020-’21” in the short-term rehab category.
