Specialty Initiative Developed by Marquis Health Consulting Services
FREEHOLD, N.J., May 5, 2022 — Further pairing its clinical excellence with enhanced communication and support for residents and their families, The Jewish Home for Rehabilitation & Nursing in Freehold became the first facility to roll out Marquis Health Consulting Services’ new Clinical Concierge Program in late 2021. Now the specialty initiative, with Annie Swartz, RN, BSN, as nurse concierge, is serving as a role model for other Marquis client facilities.
The Jewish Home’s Clinical Concierge Program facilitates seamless communication among residents, their caregivers, and the facility’s nursing and medical staff. As the nurse concierge, Swartz assists in educating residents on their medical conditions and medications. She also liaises with residents’ caregivers, providing regular updates, including changes in condition, and answering any questions they may have.
“The Jewish Home was the first to roll out this one-of-a-kind Clinical Concierge Program,” said Marquis’ Pam Montemurno, regional director of market development for the skilled nursing consulting organization. “Annie has done such a tremendous job that her approach is now being replicated at numerous other nursing homes.”
Based in Brick, N.J., Marquis Health Consulting Services is a highly skilled and vision-
driven consultant that supports Mid-Atlantic and New England facilities totaling approximately 6,000 skilled nursing and assisted living beds. Marquis is known for its continued focus on introducing innovative skilled nursing program models for its clients’ utilization.
“It’s a pleasure to help onboard Clinical Concierge Programs at other facilities and support nurse concierges in making the initiative their own,” said Swartz. “At The Jewish Home, I truly enjoy meeting new residents and their family members. I am here to assist with any questions and communicate with our facility’s talented team, further enabling us to provide excellence in care for residents.”
Swartz possesses a deep nursing background, including nearly 20 years of experience as a staff nurse at Riverview Medical Center on the Cardiac Step-Down/Medical Surgical Unit, among others. In addition, she founded a home care agency and worked as a nurse at Camp Harlam. Swartz, who is a member of the Sigma Theta Tau International Honor Society of Nursing, earned her nursing degree from Monmouth University. “Annie has such a warm, caring manner and brings extensive medical training to this expanded role at The Jewish Home,” added Montemurno.
Located at 1151 W. Main Street, the 150-bed skilled nursing facility provides post-hospital care, short-term rehab and long-term residential care. The Jewish Home is committed to serving as a healthcare resource for the families of patients and the local community. Together with Marquis, it has established a resource library of physician-curated content, addressing many topics of interest for families of patients and the larger community.
