Strong Early Outcomes at Marquis Health-Supported Centers

FLORHAM PARK/NEW PROVIDENCE/OAKLAND, N.J. (Jan. 29, 2026) — Atlantic Health System (AHS) has selected three northern New Jersey skilled nursing centers for a new Pulmonary Integrated Care (PIC) partnership aimed at improvement of post acute care and clinical management of patients with chronic obstructive pulmonary disease (COPD) and pneumonia diagnoses. Based on the initial metrics, the outcomes are encouraging, with hospital readmission rates among this patient population down nearly 7% over three months – exceeding average national performance.

The facilities, designated as “Pulmonary Centers of Excellence” by AHS, include Florham Park Rehabilitation & Healthcare Center in Florham Park, Spring Grove Rehabilitation & Healthcare Center in New Providence and Oakland Rehabilitation & Healthcare Center in Oakland. The centers all are supported by Marquis Health Consulting Services and are members of the Atlantic Health System High-Performing Network. Each has an established pulmonary rehab program with a pulmonologist and respiratory therapists on site.

Multidisciplinary and collaborative, the PIC program focuses on improving outcomes and incorporates evidence-based clinical protocols for pulmonary care. Services are suitable for both short-term rehab patients and long-term care residents. Marquis’ Jennifer Hertzog, vice president of marketing and business development, Mid-Atlantic, said Florham Park, Spring Grove and Oakland are well positioned to accommodate this distinctive new care alignment, which is a true clinical-to-clinical alliance.

“When a pulmonary patient with a COPD or pneumonia diagnosis is discharged to Florham Park, Spring Grove or Oakland from an Atlantic Health System hospital, it is much more than just a referral from AHS,” she noted. “It is the beginning of a hospital- and facility-level engagement among skilled providers to ensure continuity and consistency of pulmonary care delivery.”

The three Marquis-supported skilled nursing centers meet stringent PIC program criteria developed by AHS and Clinical Care Consulting, which provides clinical consulting services to Marquis’ skilled nursing clients. This includes key measures for training, quality scores, population tracking and care coordination, among others. Collaboration specifics incorporate clinical management, staff education, performance improvement and more.

Hertzog noted Marquis is proud of its role in bringing together a valued hospital system partner with skilled nursing resources to help address the specific challenge of high return-to-hospital rates among pulmonary patient discharges. “We are appreciative of AHS’s and our client centers’ shared commitment to providing progressive services across many specialized care areas,” she said. “And we look forward to enabling future collaborations based on developing effective solutions for real needs.”

Spring Grove Rehab & Healthcare, Oakland Rehab & Healthcare and Florham Park Rehab & Healthcare provide post-hospital care, short-term rehab and long-term residential care. In addition to pulmonary care each offers a variety of specialty rehab initiatives, such as cardiac, stroke recovery and chronic kidney disease management.
