Miami Skilled Nursing Facility Partnering with Palliative Care Specialist AccentCare in New Initiative
MIAMI (Aug. 8, 2023) — West Gables Health Care Center in Miami has introduced a new, comprehensive Chronic Illness Support model, partnering with AccentCare to integrate palliative care, pastoral support and other services with its specialized clinical programming. Located at 2525 SW 75th Street, the skilled nursing facility provides post-hospital care, short-term rehab and long-term residential care.
West Gables developed its patient-centric Chronic Illness Support program based on a platform introduced by Marquis Health Consulting Services, which supports the center.
“West Gables’ new program for chronically ill patients is truly a next-level model of care, and it is being supported by an excellent partner in AccentCare,” said Marquis’ Kate Bauer, regional director of market development. “There is great value in providing a holistic focus on both physical and spiritual wellness for patients.”
The palliative care component of West Gables’ Chronic Illness Support program is administered by a trained team of doctors, nurses and palliative specialists, and focuses on providing emotional and spiritual support to help relieve symptoms and stress. The goal is to enhance quality of life for patients and their loved ones.

These services aim to provide both comfort and peace of mind during any stage of a patient’s journey.”
West Gables’ overall initiative is specifically supporting its Chronic Kidney Disease (CKD) Management and Neuro Recovery programs, as well as its Pulmonary Rehabilitation offering, which includes full time respiratory therapy. It also weaves in disease-specific education and health coaching, multi-specialist led and coordinated care, social services support and connections to community resources, and life enrichment activities.
West Gables Health Care Center maintains a five-star rating in quality measures and staffing from the Centers for Medicare & Medicaid Services (CMS). Conveniently perched atop West Gables Rehab Hospital, it offers ease of transfer and a full spectrum of post-acute programming in collaboration with the hospital’s experienced physicians.
